Establishing Care from Day One

Jordan was just placed with a foster family, the Simons, who were unsure about his health needs and lacked medical records. Rather than go to the ER, they scheduled a same-day session with a Care Advocate to create a clear plan on what to do next:

  • How to access his Medicaid information

  • How to connect with a local provider via telehealth to refill his inhaler

  • Referrals for trusted PCPs

  • Help scheduling a first visit with a caring therapist and caregiver support

After reuniting with his first family, Jordan continued therapy with the same provider.

Virtual Mental Health Therapy

María recently moved in with her grandmother while her mom completed rehab. Her school counselor recommended therapy to help her adjust.

Since her grandmother doesn’t drive, she used her phone to connect María to Here Now Health, where they were able to:

  • Match with a therapist who understood María's unique needs

  • Receive support for both María and her grandmother during this transition

  • Get consistent care that continued even after María returned home to her mom

This ensured María had uninterrupted care during a challenging time.

Care Enablement

Deon recently turned 18 and moved out of his foster family’s home into a friend’s apartment. He connected with Here Now Health and spoke with one of our care advocates. Our Care Advocates helped him:

  • Access his Medicaid ID, understand how to use it, and find a nearby clinic that accepted his plan

  • Navigate the healthcare system, schedule appointments, and empower him to advocate for his needs

This gave Deon peace of mind as he navigated this new chapter.

As a mom, former foster parent, Court-Appointed Special Advocate (CASA), and pediatric healthcare executive, I’ve seen the life-changing impact of timely care—and the heartbreak when it’s missing. I’ve faced ER visits for minor issues, struggled to find therapists who understand the system, and witnessed how a lack of healthcare can tear families apart.
